The case submission checklist.
Everything that helps us triage a referral quickly — and an honest note on which items actually speed things up versus which can follow later. Do not hold a case waiting on a perfect packet.

Patient contact informationSpeeds triage
Name, phone, and email. We reach out to the patient within one business day of a referral to schedule.

Brief case description and your askSpeeds triage
Two or three sentences: what you are seeing, what you want from us — evaluation only, co-managed treatment, or full surgical handoff of the periodontal/implant phase.

Recent radiographs, and CBCT if availableSpeeds triage
Send what you have. Recent imaging can save the patient a duplicate exposure; if none exists, we image at the consult.

Periodontal charting
Pocket depths, recession, mobility, bleeding points — whatever your last charting captured. It shows us trajectory, not just a snapshot.

Restorative goalsSpeeds triage
For implant and crown-lengthening cases especially: the planned restoration, margin positions, and your timeline. We place surgery in service of your prosthetic end-point.

Urgency notesSpeeds triage
Flag anything that should move the case up the queue — see the urgent referrals page for what belongs on the phone instead.

Relevant medical history
Anticoagulants, diabetes control, immune status, bisphosphonate or antiresorptive exposure, smoking status. These change surgical planning and sedation decisions.

Referring-office questions
- What if I don't have all of these records?
- Send the referral anyway. Patient contact information and a two-sentence description are enough to start — we gather what is missing at the consultation. A complete packet speeds triage; an incomplete one should never delay care.
- How do I send the referral?
- Call the office at (202) 659-3500 and we'll tell you how to send the packet directly to our office. Please don't send patient details through the website form. Urgent cases: call first; the phone moves faster.
- What happens after I refer?
- We contact the patient within one business day to schedule. After the consultation, your office receives a written summary with findings, the planned treatment, and your role in the restorative phase. Urgent findings prompt a direct call.
